A miniature coffin containing muti and a small wooden cross were left at the front door of a Delft ANC member in an apparent death threat.
Godfrey Modirwa, 47, ran outside when he heard his wife screaming and saw the items outside his house on Wednesday.
Police are investigating a case of intimidation, but officers first told Modirwa that they did not believe a crime had been committed.
Delft police spokesman Captain Joe Wilson said they were treating the matter seriously.
Modirwa, who was planning to run as ward councillor for his area, said his name and the number 12 were written on the cross. He believes the 12 refers to the time the people who left the items at his house planned to kill him.

Using a shovel, he opened the lid of the box to find a white sheet wrapped around muti made of sea shells and mussels.
"This is definitely a death threat and I want to know who is behind it. I am scared for my family's safety and until we know who did this and why, I cannot relax," said a shocked Modirwa.
Modirwa has received several telephonic deaths threats over the past three months and believes he is being targeted after questioning the local ward council on its policies.
He considered running for the position of ANC ward councillor.
"I was planning to run for council but then I received several death threats over the phone saying that if I tried to go against the council they will kill me and my family.
"I am still willing to run for the position of ward councillor but only after we know who was behind this."
Wilson said police later returned to the scene to collect the coffin and were treating the case as a priority.
"We've noted the previous death threats and sent a specialist out to the scene to try and find some clues. At this point we don't have any leads but are hoping something will come from the forensic report."
Modirwa would not comment on whether the death threat could have come from a council member or another ANC member seeking the ward councillor position.
He said he would wait until the investigation was finalised before saying anything on the matter.
